The Adult Day Care Program provides quality services as an alternative to institutional care, in an environment where each participant's quality of life is enhanced by focusing on their uniqueness.
Who we serve:
The program is available for elderly, age 60 years and older who suffer health impairments such as sensory loss. Loss of mobility (caused by strokes, amputation, disease such as arthritis, Parkinson’s, poor circulations, injuries and blindness), incontinence, obesity and diabetes.
How to participate:
Profile and Referral form and assessment must be initiated by Case Management Services to determine eligibility prior to an individual being admitted into the program.
The Adult Day Care Program provides quality care for up to 10 hours. Monday through Friday, with activities that are planned to fit individual capabilities.
- Planned activities are designated to be social, educational, recreational, and therapeutic
- Lunch and snacks are provided daily
- The activities are determined by the participants needs, abilities, and interests and are performed in a professional courteous, safe and caring manner
