Description:
To provide basic educational, preventive and treatment services to the children of Guam below the age of 17 years as mandated by Public Law 12-130 and to provide emergency dental care to adults 55 years or older for the relief of pain and/or infection as mandated by Public Law 19-10.
To educate the general public on dental health measures, with special emphasis of the optimal use of prevention methods to reduce the incidence of dental disease. These methods include the proper use of fluorides, pit and fissure dental sealants, and adoption of personal oral hygiene measures designed to reduce or prevent dental caries and periodontal disease.

The Dental Program provides free basic dental services to children under 17 years old and emergency care to senior citizens 55 years and older. Patients must meet the income guidelines set forth by the Department in order to qualify for these services. All patients with private dental insurance will be referred to private providers even if they meet the income criteria. Patients requiring services beyond that provided by DPHSS will be referred to private providers.
Eligibility Policy
1. Must have no dental insurance
2. Receives public assistance (Medicaid, MIP, Food Stamps, WIC) or
3. Meets low income guidelines (based on 125% of the State of Hawaii’s Poverty Income Guidelines)
Documents Required:
- Photo ID of parent or legal guardian
- Birth certificate of child
- Guardianship papers (if apply)
- Current Medicaid or MIP card (if apply)
- Two latest check stubs from each parent or legal guardian who support the child
- Copy of the most recent income tax return filed from each parent or legal guardian showing the child as a dependent
Guam Fluoride Varnish Program
The Fluoride Varnish Program is open to children under five years old who are enrolled in the Head Start Program, attend daycares, and receive immunizations at outreaches.
