ADULT DAY CARE PROGRAM
 
Adult Day Care Program is a non-residential facility that supports the health,nutritional, social, and daily living needs of adults in professionally staffed, group settings. The centers provide meaningful activities, and general supervision, assistance to maintain independency and provide respite to their families.

ELIGIBILITY
 
Adult Day Care – Central is available for adults, age sixty (60) years and older, who suffer health impairments such as sensory loss, loss of mobility (caused by strokes, amputation, diseases such as polio, arthritis, Parkinson’s, poor circulation, spastic condition, paraplegic, injuries, and blindness) incontinence, epilepsy, obesity, and diabetes;

Adult Day Care – North (Dementia)
adults, age sixty (60) years older diagnosed with Dementia and/or Alzheimer’s;

Adult Day Care South (Inalahan)
adults, age sixty (60) years and older mix populations and/or combination who resides in southern part of Guam.


 
REQUIREMENT FOR ADMISSION
 
Adult Day Care Program required Physician Certification and Client Medical Report certifying with two (2) or more, not more than six (6) Activities of Daily Living (ADL) impairments by their Primary Physician.

ADULT DAY CARE PROGRAM
Adult Day Care centers vary tremendously in terms of services and types of
clients who benefits from services as follows. For example, adult day
care programs may provide:
 
 social activities
 counseling
 meals / special diet
 specialized care for dementia
 transportation
 recreation
 medical help
 emotional support
 mental stimulation
 exercise
 education
 assistance with walking, eating, taking
medications, bathing
 health monitoring - blood pressure,
blood sugar levels, food/liquid intake,
weight.


TELEPHONE ASSURANCE PROGRAM
Telephone Assurance Program (TAP) is a program that makes regular telephone calls to
ADC seniors to check on their well-being and give an opportunity to socialize and receive
updates information. This program is activate during the time of pandemic
emergency that client was place on temporary homebound to ensure their safety.
TAP’S main purpose is to ensure ADC seniors are in good condition it allows the
staff to communicate and if any concerns are discussed if will be addressed by the
management and forwarded to a Registered Nurse and seniors are assigned caseworkers at
CMS program.
 
COVID-19 SERVICE OPERATION
 ADCP will do (or conduct) virtual health checks for employees and participants (for example, symptoms and temperature screening) before they arrive at the center.
 ADCP will ensure confidentiality will be implemented before collecting health- related information from staff or participants.
 Limit non-essential visitors (the program will get primary caretaker approval on visitors)
 Ensure all persons entering the facility adhere to the same safety protocols as the staff and participants.
 Keep a daily entry log with contact information for all staff and participants.

Activities and services offer seniors companionship and enjoyment. But all day care centers offer seniors scheduled programs that build consistent routine as follows: 

Time Activity
8:00-9:00: Welcoming and Morning walk
9:00-11:00: Current Events, activities (arts & crafts)
11:30-12:30: Lunch
12:30-2:00: Relaxation (Therapeutic activities)
2:00-3:00: Cognitive Stimulation
3:00-4:30: Happy Hour (games, singing, etc)
4:30-5:30: Prepare for transportation plan for pick up.
